The Patient Portal is your resource to quickly communicate with your Provider. You can use the Patient Portal to complete patient forms, update your health history, view visit notes, and lab results.
We are in network with most major health insurance providers in your area. Click the above link for a complete list of insurance networks that we participate in. If you do not see your insurer listed, please notify our office. In addition to insurance, we also accept cash payments at comparable rates to insurance reimbursement. Please contact us for cash prices.
We are accepting new patients. In order to provide patients with low wait times and convenient care, we encourage prospective patients to Pre-Enroll with Johnson Health Solutions. Pre-Enrollment is a simple process where patients complete insurance and health history paperwork at their convenience prior to appointments. This simple process helps reduce wait times when care is needed. To have a staff member reach out to assist in the completion of the Pre-Enrollment process, please click the link above and complete the patient form.
All appointments are scheduled in advance with adequate time for your needs. We will make every attempt to honor your appointment time request, as we do understand the value of your time and the need to be seen. Please understand that unpredictable delays may occur due to deliveries or unexpected emergencies. If timing is critical, you may reschedule. If you have to cancel your appointment, we ask you notify us as soon as possible.
Patients are encouraged to call anytime they have questions or concerns. In order for us to provide the best quality of healthcare, we do ask patients to call our triage Nurse during regular office hours when your chart is available for reference.
If our Nurse is assisting another patient, please leave a message including your full name, date of birth, phone number and the best time of day to return your call. We will return calls as soon as possible.

The Financial Policy and Disclosure is to help us provide the most efficient and reasonable health care services. Therefore, it is necessary for us to have a Financial Policy and Disclosure stating our requirements for payment for services provided to patients
